TI Evaluation of effectiveness of treating various form of syphilis in
HIV-infection with large doses of intramuscular benzylpenicillin.

AB PURPOSE: To determine effectiv therapeutical approach to syphilis in
HIV-infected patients. METHODS: In 14 patients (female-1, male 13 aged
from 21 to 42; primari siphilis-1 (re-infection), secondary-7
(re-infection in 1), latent-6 (re-infection in 1), the diagnosis of
HIV-infection was confirmed with ELISA and Western blot, of
syphilis-with detection of T. Pallidum express-method, complement
linkage reaction (CLR) with both antigens. FTA-ABS, FTA-200,
immobilization reaction (IR), and when necessary--with histological
studies. The method of therapy made by the authors was use:
benzylpenicillin im by 4 million IU every 4h for 14 days (course dose
336 million IU). Clinikal and serological control after the finishing of
therapy was performed for 1.5 yrs: CLR-bimonthly; FTA-200, FTA-abs and
IR-sixmonthly. In 2 (1-latent, 1-secondary syphilis) the therapy was
censed due to acute pain in the sites of injections. RESULTS: The
tolerance of the method was 80%. In primary syphilis (re-infection) the
decline in positive intensiveness and in titer of CLR was seen with the
absence of dynamics of FTA-200, FTA-abs and IR. In 4 patients with
secondary syphilis CLR became negative in average 4 mos after the
finishing therapy, in 2-decline in positive intensiveness and in titer
of CLR was seen, 1 (re-infection) became seroresistent. In
latentsyphilis seronegativation in CLR was seen in 3 after 5.5 mos, the
decline in positive intensiveness and in titer of CLR was seen in 1 and
seroresistence--in 1. In secondary and latent syphilis FTA-200, FTA-abs
adynamic and IR declined in average by 40% for 1.5 yrs. CONCLUSIONS: The
large doses method of therapy was effective in 83% HIV-infected patients
with varios forms of syphilis. The posttreatment seroresiitens coincided
with reinfection of syphilis (2 from 3 pts). The effective therapy of
syphilitic seroresistens in HIV-infection remains to be determined.
